Move your assets to Fidelity
- Find out what your former employer requires to do a rollover. This likely includes account numbers and balances. If you don’t have this information, a Rollover Specialist can contact your former employer or plan administrator while you are on the line.
- Fill out the distribution form that your former plan will send you. Be sure to ask the plan to transfer your assets to your new Fidelity account number.
- If your former employer or plan administrator requires Fidelity to complete a section or sign the distribution form, please mail the distribution form and a recent plan statement to Fidelity (see address below) and we will complete the required section and mail the paperwork to the plan. Remember to include the mailing address for the plan administrator.
