I will print and read the following Fact Kit, Participation Agreement, Customer Agreement, and Account Application (together, the "Agreements"), and
I will not alter the terms of the Agreements as originally set forth by you.

I acknowledge that any deviation from the Agreements' original terms shall be void, and I shall be bound by the terms of the original Agreements as set forth by you. I agree that the Trust, FBS and/or NFS may void the Agreements in the event there are reasonable grounds to believe the original Agreements have been altered.

If you are opening accounts for more than one beneficiary, please print and complete a separate account application for each beneficiary.
