Power of attorney gives a designated person the ability to act on behalf of the account owner. This includes, but is not limited to, placing orders in an account, requesting disbursement of funds, and changing an address.
There are two alternative forms for establishing durable power of attorney on your Fidelity account(s):
Durable Power of Attorney Form
If you have not executed a durable power of attorney document, use the appropriate Durable Power of Attorney Form to add or update this information on your Fidelity account(s). For your convenience, this form serves as your durable power of attorney document, and incorporates the power of attorney affidavit and indemnification. This form provides for power of attorney on your eligible Fidelity accounts only.
Fidelity Durable Power of Attorney Form (PDF) – includes 529 Accounts
Fidelity Freedom Lifetime Income, Fidelity Growth and Guaranteed Income, Fidelity Income Advantage, Fidelity Personal Retirement Annuity and Fidelity Retirement Reserves Durable Power of Attorney (PDF)
Durable Power of Attorney Affidavit and Indemnification Form
If you currently have a fully executed durable power of attorney document, use the appropriate Durable Power of Attorney Affidavit and Indemnification Form. Simply have your Attorney-in-Fact complete, sign, and return the form along with a copy of your durable power of attorney document.
